A home care assessment is an opportunity to talk openly about daily life, current challenges and the kind of support that would make a positive difference. A little preparation can help everyone leave the conversation with a clearer picture.
Think about a typical week
It can be useful to note what happens from morning to evening, including meals, medication routines, appointments, mobility, personal care and social activities. Consider which moments feel manageable and where support would help.
Bring the details that matter
- The person’s preferred routines and communication needs.
- Any current support from family, friends or professionals.
- Important access information and home-safety considerations.
- Questions about timing, continuity, reviews and how changes are handled.
Make space for the individual’s voice
Family input can be valuable, but the person receiving care should remain central to the conversation wherever possible. Their goals, choices and consent should guide what happens next.
An assessment should lead to a clear, person-centred care plan before support begins. If circumstances change, the plan can be reviewed and updated.
